Hey,
how does a 20 mm hub work ... do they all use sealed bearings ? Is the only thing stopping the axle from moving horizontally, the forks ?

Well take a quick release hub. Make the hole 20mm big. Then make the Axle 20mm thick. (and depending on the axle, remove the levers to tighten) .
Plus u need a fork that supports 20mm.
It works pretty much like a QR hub but bigger.
And no, they are not ALL sealed.

20mm front hubs generally consist of the hub body/flanges, sealed cartridge bearings pressed in with an internal diameter of 20mm and then two spacers on either side.
Open bearings cup/cone style dont have a place here. So they all run sealed cartidges.
The bearings stop it from going left to right.
